Hello MW HOCKEY Players and Parents!

Our Season End Surveys completed by parents and players are a valuable form of feedback for the directors and for the coaches. MW HOCKEY enlists an online survey to acquire feedback. This is a tremendous way to get the information our coaches and directors desire in a confidential, timely manner.

The survey is easy to take. It should take no more than 10 minutes from start to finish. It is easy to navigate. There are questions for the parents to complete. There are questions for the players to complete.

It is important that at least one parent and EACH player complete a survey for their team.

This method of data collection is strictly confidential. MW HOCKEY guarantees your privacy by not having the ability to collect personal information or the location from where a survey was completed. The only data collected is the answers you give in the survey. You will have the opportunity to offer your name and email address if you so desire, but this information cannot be tracked by MW HOCKEY. This is important to us so that you can be comfortable in providing honest feedback that will make our program better.

Please take 10 minutes for your player and yourself to complete this survey. We want and need to hear the GOOD, the BAD and the UGLY. Do not think of this as a waste of time if you have only good things to state. State them. Do not think that any negative answers or comments will be shuffled aside. They will be taken seriously. Only by getting input from all peoples involved, will our association continue towards excellence. If you do not have internet access, please contact your team manager. Arrangements will be made to have your feedback collected.
