Millwoods Hockey will be once again offering Online Player Registration.  Pre-printed player registration forms will be mailed shortly.

• Next Registration Night - August 18, 2010 - Leefield Community Hall (7910-36 Ave) from 7-9 PM.
• NO Community League Membership Cards will be given on Registration Nights. Please click here for more information.

• Please ensure all the information on your child’s form is correct. (If there is an item left blank please make the necessary addition or changes.)

• Please ensure you have signed and dated the form.

New Players: Must include the following documentation

• Alberta Health Care
• Birth Certificate
• Proof of Residency( Drivers license, Utility bill)
• 2010 - 2011 Community League Corporate Rate Family Membership (Mandatory)

 Payment Options:

• Personal cheque along with any Bingo Vouchers. (Cheque payable to MWHA)
• Visa/MasterCard : include card #, name on card and expiration date
• Fees can be post-dated with half fees due on June 16, 2010 and the remainder no later than August 18, 2010.
